SLIDING SCALE FEE SCHEDULE
The usual rates are $120 for the diagnostic visit, and $100 for each subsequent visit for a licensed counselor. For unlicensed counselors, our fee is $110 for the diagnostic visit and $90 for each subsequent visit. For those who may have difficulty paying and do not have insurance, we offer a sliding scale based on gross household income. If you have been offered a reduced fee based on income, please bring a copy of a 1040 or a federal tax form to verify income.
Income of $ 50,000 and under
1st visit $ 75 - $ 85
subsequent visits $ 65 - $ 70
Income of $60,000 and up
1st visit $ 100 - $110
subsequent visits $ 85 - $ 95
Income of $ 50,000 and up
1st visit $ 85 - $95
subsequent visits $ 70 - $80
Income of $70,000 and up
1st visit $ 110 - $120
subsequent visits $ 90 - $100
